This sample is a sales asset: it shows how Shortlist builds a named target list from CMS + state licensure + SOS/About signals, and where the public record stops.
1. CMS Provider Data Catalog — Home Health Care Agencies (6jpm-sxkc): pulled VA (237) and NC (166) certified agencies; filtered PROPRIETARY; used quality-of-care star and the timely-initiation denominator as a crude episode/volume proxy (labeled as estimate everywhere).
2. Medicare Care Compare detail URLs for each shortlisted CCN (address, phone, ownership type, star).
3. NC DHSR Acute & Home Care facility listings — *Home Health Services all* XLSX (as of 08/2026) and *Home Health with Hospice* list: legal name, DBA, site city, license #, facility contact name/phone.
4. Company websites / About / leadership pages and reputable local or trade press for owner/founder names.
5. NPI / OpenNPI / BBB / OpenCorp-style directories for authorized officials and registered-agent clues when About pages were silent.
6. Attempted Virginia SCC CIS entity search and VDH OLC facility portal for officer/license detail — interactive portals; not usable as bulk download this pass (see Gaps).
We did not invent phones, emails, or owners. Blank owner_or_principal means the public pass did not support a name.
- Virginia SCC officers — CIS is interactive; automated pulls failed this pass. Several VA rows have strong About/NPI signals but no confirmed officer slate.
- VDH Office of Licensure & Certification — licenses Home Care Organizations, but there is no clean statewide public CSV equivalent to NC’s DHSR XLSX. Medicare certification ≠ full VA HCO universe.
- True census / EBITDA / payer mix — CMS denominators are not patient census or revenue. Staff counts from About pages are self-reported.
- NC Certificate of Need — Medicare HH slots are scarce; the licensed universe is dominated by nationals, hospital systems, and a few scaled regionals. True small independents are thin — this sample’s NC count is honest, not padded.
- JS-heavy sites (e.g., HealthView, Esprit) returned limited About text without a browser session.
- Brand vs legal entity — several operators use DBAs that differ from the CCN label or state licensee (e.g., Continuum II → Cardinal Hospice Care on NC license; 1st Care vs Commonwealth Home Health). Entity maps need a paid diligence pass.
- Equity % — “CEO,” “founder,” and “registered agent” are not proof of controlling ownership.

Supporting rows in targets.csv include additional VA independents (Valley Ridge, Pavilion, Five Star, Nayar, Kind Hearts / Eastern Shore, etc.) plus NC PHC, Continuum II / Cardinal, and Well Care (family-owned but scaled — context / upper-bound, not a classic search-fund bite).
Treat the CSV as a screening queue, not a final CIM list. Re-verify every CCN on Care Compare, pull SCC/SOS officers the morning of outreach, and skip anything that resolves to a national parent.
